Health Care Transition Resources - the CHAT Project

The Health Care Provider’s Guide to Helping Youth Transition from Pediatric to Adult Healthcare (and its companion guidebooks and toolkits for youth and their families) was prepared by the North Carolina Division of Public Health, Children and Youth Branch, in collaboration with the Mountain Area Health Education Center (MAHEC), the Exceptional Children’s Assistance Center, and the Center for Independent Living. These materials were funded by the Carolina Health and Transition (CHAT) Project, a state initiative to improve the quality and accessibility of health care transition services for youth, their families, and their medical providers throughout the state.

MAHEC is pleased to provide these guidebooks and tools, along with other project educational materials and resources. All materials are available for download and distribution for educational purposes. If users adapt the materials, they are required to cite the source document(s).
