New Patient Information

If you would like to become a patient of the MAHEC Family Health Center, we will need some basic information from you. With this information we will set you up in our computer system as a patient and assign you to a physician in our practice. Then when you need to see the doctor, all you will need to do is call us to make an appointment. Asheville location 828-257-4730; Cane Creek location 828-628-8250.
