Membership Application Form
All are welcome to use the library space, computers, and books/CDs/DVDs in the library at no cost, courtesy of MAHEC.
Membership in MAHEC Library and Knowledge Services includes the privilege of borrowing books/CDs/DVDs. Books/CDs/DVDs may
be borrowed from our collection for 3 weeks, with 2 renewals allowed if no hold has been placed on borrowed items. Items we
borrow from other libraries for you are subject to the loan period and renewal schedule set by the lending library.
Return incentive (overdue) fees, intended to encourage timely return of items, accrue at the rate of $.50 per day per item.
If overdue fees total $20 or more, circulation privileges are revoked until fines are paid.

The AHEC Digital Library
offers access to online databases, e-journals, e-books, continuing education, special collections and much more. Certain
licensed resources (many of the full-text journals, books and databases) are available only through paid subscriptions to the
AHEC Digital Library.
